What Do All the Forms Mean?

EWB-USA has some forms that we need chapters to submit prior to starting on a project travel and some forms are there to help guide chapters while on the project.  

Forms We Need:
Basically, EWB-USA needs to know when a chapter is planning to do a new project (even within the same community in which a chapter is working) or when the chapter plans to travel to the community to assess or implement some part of a project.  We'd appreciate it if you could help us out and fill out the appropriate form and send in to EWB-USA to Projects, or send us any questions if you're confused on any part.

501 - New Project Application Form	Please fil this out and send this to EWB-USA if you're planning on a completely new project in a new country for your chapter.  Once this application is approved by EWB-USA, then your chapter can start on the project and filling out more forms.
501B - Continuing Project Form	Please fill this out and send to EWB-USA if you're planning a new project in the community you've been working.  (i.e. Your chapter just completed the installation of a water system and now the community wants lighting for the school)
507 Section A 605 Waiver 608 Health	Please fill in Section A and send in waivers/health forms for each person traveling if you're planning to conduct an assessment in your community.   This should be sent in after the project application (501) is approved.
507 Section B	Please fill in Section B with information about your assessment trip after you've completed your assessment trip.
507 Section C 605 Waiver 608 Health	Please fill in section C with all the relevant technical and cultural information, and submit a waiver and health form for each person traveling if you're planning to implement a project in your community.
507 Section D	Please fill in Section B with information about your implementation trip after you've completed your assessment trip.
